How to Position Your Arm When Taking Blood Pressure

Rest your arm on a table so the cuff sits level with your heart. A lap or dangling arm can add 4 to 7 mmHg. Here's how to get arm position right.

Support your arm on a table or desk so the middle of the cuff sits level with your heart, and let the surface hold the weight so your muscles stay relaxed. That single detail changes the number more than most people expect. When your arm rests in your lap or hangs at your side instead, the reading comes out too high, and the error is large enough to move you from one blood pressure category to the next.

A 2024 randomized trial published in JAMA Internal Medicine put exact numbers on this. Compared with an arm supported on a desk at heart level, resting the arm in the lap overestimated systolic pressure by about 3.9 mmHg, and letting it hang unsupported at the side overestimated it by about 6.5 mmHg. For people who already run high, the gap was wider still: their systolic reading averaged around 9 mmHg higher with a dangling arm. The American Heart Association puts arm support at heart level among its core rules for a reason, and this is it.

The short answer: supported, at heart level, relaxed

Three things have to be true at once. First, the arm is supported by a firm surface, not held up by your own muscles. Second, the middle of the cuff sits at the level of your heart, roughly at the mid-point of your breastbone. Third, the arm is relaxed, palm up, with nothing tensing it. Miss any one of these and the number drifts upward. The most common miss is height: an arm resting in the lap sits below heart level, which raises the reading, while an arm propped too high can nudge it the other way.

Those numbers come from the ARMS crossover trial. A 6.5 mmHg overestimate is the difference between a systolic of 123 and 130, or between 133 and 140. Week after week, that can be the line between "normal" and a category that prompts a conversation with your doctor.

Why height matters so much

Blood pressure is measured against gravity. When the cuff sits below the level of your heart, the column of blood between your heart and the cuff adds its own weight to the pressure, so the monitor reads high. Raise the cuff above heart level and the opposite happens, giving you a falsely low number. The reference point that matters is the middle of the cuff lined up with the middle of your heart, which is close to the mid-sternum when you sit upright.

This is why the lap is a trap. It feels natural and relaxed, but for most people sitting in a chair the lap sits several inches below heart level, enough to push systolic up by roughly 4 mmHg on its own. Resting the arm on a table of about the right height fixes the geometry without you having to think about it.

Support the arm, don't hold it up

Height is only half of it. The other half is muscle. If you hold your own arm out or keep it raised using your shoulder, the sustained muscle contraction, called isometric exercise, raises your blood pressure while you do it. The fix is to let something else carry the weight. Rest the forearm flat on a table, or on the arm of a chair or a firm cushion, so your arm goes limp. You should be able to relax your hand completely, palm facing up.

A quick self-check: once the cuff is on and you are seated, drop your shoulders and open your hand. If you feel any tension holding the arm up, it isn't fully supported, and the reading will run high. Getting the cuff placed correctly on a bare arm matters just as much as the height you rest it at.

Arm position is not the whole posture

Arm position works together with the rest of your body. Which arm, and does it change position?

Use the same arm each time so your readings stay comparable, and if a doctor has told you one arm reads higher, use that one. The positioning rules do not change between left and right: support whichever arm you use with the cuff at heart level. Switching arms day to day is one more reason readings seem to jump around when nothing about your health has changed.
