Blood Pressure Over Clothes or Bare Arm: Which Is Right?

Guidelines say measure on a bare arm, not over clothing, because a thick or bunched sleeve can skew the reading. Here's how big the effect really is.

Put the cuff on a bare arm. Every major measurement guideline, including the American Heart Association's, says the cuff should sit against bare skin, not over a sleeve. A thin t-shirt usually changes the number by only a few mmHg, but a thick sweater or a tightly bunched rolled-up sleeve can shift it more, and the direction is unpredictable. Since you can't reliably guess which way clothing pushes your reading, the safe habit is to bare the arm every time.

The reason comes down to what a cuff actually does. It squeezes the arm and listens for the pulse returning as it deflates, so anything between the cuff and your skin can muffle or distort that signal. Why clothing changes the number at all
